No one should authorize safe work before being certain that it can be done safely. Before beginning work and when it is finished, all safe work permits must be signed by the permit issuer and the recipient.

Safe work permits are often filled out in triplicate or in two copies. When a duplicate system is employed, the worker or department performing the work holds one copy of the permit while the issuer keeps the other at the work site.
